Tucatinib Inhibits Renal Transporters OCT2 and MATE Without Impacting Renal Function in Healthy Subjects.

Tucatinib is a potent tyrosine kinase inhibitor selective for human epidermal growth factor receptor 2 (HER2) approved by the US Food and Drug Administration for the treatment of HER2-positive metastatic breast cancer and in development for other HER2-positive solid tumors. Modest, reversible serum creatinine (SCr) elevations have been observed in tucatinib clinical trials. SCr is conveyed by the renal drug transporters organic cation transporter 2 (OCT2) and multidrug and toxin extrusion protein 1 (MATE1) and 2-K (MATE2-K) and can increase in the presence of inhibitors of these transporters.

La seguridad en el uso de Rustia formosa es un tema de extrema cautela debido a la complejidad química de sus componentes. Según el análisis de sus cavidades secretoras, la planta contiene una mezcla compleja de al menos 75 componentes, predominantemente sesquiterpenoides [PMID 21669647]. Esta composición química implica que el uso de la planta puede provocar reacciones sistémicas impredecibles. En cuanto al embarazo y la lactancia, no existe evidencia científica que garantice la inocuidad de estos sesquiterpenoides en el desarrollo fetal o neonatal; por lo tanto, su uso está estrictamente contraindicado en mujeres gestantes y lactantes, ya que los metabolitos podrían atravesar la barrera placentaria o ser excretados en la leche materna, con riesgos potenciales para el desarrollo del infante. Para niños menores de 12 años, el riesgo es aún más elevado debido a la inmadurez de sus sistemas enzimáticos de desintoxicación, lo que podría resultar en toxicidad aguda.
