Background. Terminalia arjuna is a popular Indian medicinal plant with its bark been used for over centuries as cardiotonic. The bark has been found to contain several bioactive compounds including saponins and flavonoids. A number of experimental and clinical studies have been conducted to explore therapeutic potential of Terminalia arjuna in cardiovascular ailments specially in patients of coronary heart disease. A number of narrative reviews have been done but no systematic review has been conducted to date.

La seguridad de Terminalia arjuna es un tema de investigación continua, ya que, aunque cuenta con una larga historia de uso tradicional en el subcontinente indio, la evidencia científica sobre su seguridad a largo plazo sigue siendo limitada. En cuanto al embarazo y la lactancia, no existen estudios clínicos rigurosos que determinen la seguridad del uso de la corteza de Arjuna en mujeres gestantes o lactantes. Debido a que el desarrollo fetal y la transferencia de compuestos a través de la leche materna son procesos altamente sensibles, se recomienda evitar su consumo durante estas etapas para prevenir cualquier riesgo potencial no identificado. Para niños menores de 12 años, la seguridad no ha sido establecida; los sistemas fisiológicos en desarrollo son más vulnerables a los efectos de los compuestos bioactivos como los triterpenoides y glicósidos, por lo que su uso en la población pediátrica debe evitarse a menos que un profesional médico lo indique específicamente. En términos de interacciones farmacológicas, se deben tener precauciones críticas. Al ser un agente con efectos hipotensores (que reduce la presión arterial), el uso conjunto con fármacos antihipertensivos puede provocar una hipotensión excesiva (presión arterial peligrosamente baja).
