La seguridad en el uso de Armoracia rusticana (rábano picante) es un factor crítico debido a su alta concentración de compuestos azufrados reactivos. En el caso de mujeres embarazadas y en periodo de lactancia, no existe evidencia científica suficiente que garantice la seguridad del consumo de extractos concentrados de raíz; por lo tanto, se recomienda evitar su uso terapéutico. Los isotiocianatos (ITC), productos de la hidrólisis de glucosinolatos, pueden atravesar barreras biológicas y no se conocen sus efectos sobre el desarrollo fetal o la transferencia a través de la leche materna. En niños menores de 12 años, el uso debe ser extremadamente cauteloso o evitado, ya que su mucosa gastrointestinal y vías respiratorias son más sensibles a la pungencia irritante de los compuestos de azufre, lo que podría provocar espasmos o irritación severa. Respecto a las interacciones farmacológicas, la evidencia sugiere riesgos específicos. Debido a su potencial actividad sobre la alfa-glucosidasa (como se indica en el estudio de la capacidad antidiabética [PMID 32452324]), el consumo de Armoracia podría potenciar el efecto de fármacos hipoglucemiantes como la metformina o la gliclazida, aumentando el riesgo de hipoglucemia.
