La seguridad de Pinellia ternata es un asunto de extrema importancia debido a su perfil toxicológico intrínseco. El rizoma crudo no debe ser administrado por vía oral, ya que contiene componentes altamente irritantes como cristales de oxalato de calcio en forma de aguja (NCOCs) y lectinas (proteínas), los cuales pueden causar irritación severa de la mucosa oral y faríngea, así como efectos sistémicos graves [PMID 36087845]. En cuanto al embarazo y la lactancia, el uso de Pinellia está estrictamente contraindicado. La evidencia científica indica que el rizoma posee toxicidad gestacional y potencial teratogénico (capacidad de causar malformaciones congénitas en el feto) [PMID 36087845]. Debido a que los mecanismos de transferencia de compuestos a través de la placenta y la leche materna no están completamente definidos, el riesgo de daño al desarrollo fetal o neonatal es inaceptablemente alto.
