This overview of systematic reviews (SRs) aims to evaluate critically the evidence regarding the adverse effects of herbal medicines (HMs). Five electronic databases were searched to identify all relevant SRs, with 50 SRs of 50 different HMs meeting our inclusion criteria. Most had only minor weaknesses in methods. Serious adverse effects were noted only for four HMs: Herbae pulvis standardisatus, Larrea tridentate, Piper methysticum and Cassia senna. The most severe adverse effects were liver or kidney damage, colon perforation, carcinoma, coma and death.

Seguridad y Precauciones
La seguridad en el uso de Salvia dorrii es un tema que requiere cautela, dado que la evidencia científica sobre sus compuestos específicos, como el salvidorol (un diterpeno nor-abietano), es limitada y se centra principalmente en su estructura química aislada [PMID 16458943]. En cuanto al embarazo y la lactancia, no existen estudios clínicos que determinen la seguridad del consumo de esta especie en mujeres gestantes o lactantes; por lo tanto, se debe evitar su uso debido al riesgo potencial de efectos teratogénicos (malformaciones fetales) o alteraciones en el desarrollo del lactante a través de la leche materna. Para niños menores de 12 años, el uso no está recomendado, ya que los sistemas metabólicos y enzimáticos en desarrollo pueden ser más sensibles a los diterpenos y otros metabolitos secundarios de la planta, y no se conocen las dosis seguras para poblaciones pediátricas. Respecto a las interacciones farmacológicas, el uso de Salvia dorrii podría interferir con diversos medicamentos. Si se consume junto con fármacos anticoagulantes como la warfarina, podría existir un riesgo de alterar la cascada de coagulación, incrementando el riesgo de hemorragias. En el caso de la metformina o fármacos antidiabéticos, podría haber un efecto sinérgico que resulte en hipoglucemia (niveles peligrosamente bajos de azúcar en sangre).
