Adverse effects of herbal medicines: an overview of systematic reviews.

This overview of systematic reviews (SRs) aims to evaluate critically the evidence regarding the adverse effects of herbal medicines (HMs). Five electronic databases were searched to identify all relevant SRs, with 50 SRs of 50 different HMs meeting our inclusion criteria. Most had only minor weaknesses in methods. Serious adverse effects were noted only for four HMs: Herbae pulvis standardisatus, Larrea tridentate, Piper methysticum and Cassia senna. The most severe adverse effects were liver or kidney damage, colon perforation, carcinoma, coma and death.

Los resultados de las revisiones de este tipo de estudios indican que la planta tiene potencial para efectos antioxidantes, neuroprotectores, antifibróticos, antiinflamatorios y antineoplásicos. En este contexto, se clasificaron los efectos en menores, moderadamente severos y graves.

Seguridad y Precauciones

En relación con el embarazo y la lactancia, no existe evidencia científica suficiente para determinar la seguridad del consumo de Salvia (Salvia poculata). Debido a que diversas especies del género Salvia pueden poseer efectos sobre el sistema hormonal o actividad farmacológica no cuantificada, se recomienda evitar su uso durante el periodo gestacional para prevenir posibles riesgos de desarrollo fetal. En el caso de la lactancia, la falta de estudios sobre la transferencia de compuestos activos a través de la leche materna obliga a mantener una postura de precaución, ya que no se puede garantizar que los metabolitos de la planta no afecten al lactante. Para niños menores de 12 años, el uso de esta planta está estrictamente desaconsejado. Los sistemas fisiológicos en desarrollo, particularmente el sistema nervioso y el sistema endocrino, son altamente sensibles a compuestos bioactivos, y la ausencia de estudios de toxicidad pediátrica hace que cualquier dosis represente un riesgo impredecible. En cuanto a las interacciones farmacológicas, se debe tener especial cuidado con la warfarina (anticoagulantes); si la planta posee propiedades que afecten la coagulación, podría potenciar o inhibir el efecto del fármaco, aumentando el riesgo de hemorragias o trombosis.
