El uso de Trachyspermum ammi (comúnmente conocida como ajwain) requiere una precaución extrema en poblaciones vulnerables debido a su potente actividad farmacológica. En el caso de mujeres embarazadas, el uso de esta planta está estrictamente contraindicado debido a sus propiedades abortifacientes documentadas. El mecanismo de acción implica la estimulación de contracciones uterinas, lo que incrementa el riesgo de aborto espontáneo o parto prematuro. En el periodo de lactancia, aunque se menciona tradicionalmente como galactogoga (sustancia que puede aumentar la producción de leche), no existe evidencia clínica suficiente para garantizar la seguridad del lactante. La transferencia de compuestos volátiles como el timol a través de la leche materna podría alterar el sistema digestivo o neurológico del bebé de manera impredecible. Para niños menores de 12 años, se debe evitar el uso de extractos concentrados o aceites esenciales de la planta.
