Adverse effects of herbal medicines: an overview of systematic reviews.

This overview of systematic reviews (SRs) aims to evaluate critically the evidence regarding the adverse effects of herbal medicines (HMs). Five electronic databases were searched to identify all relevant SRs, with 50 SRs of 50 different HMs meeting our inclusion criteria. Most had only minor weaknesses in methods. Serious adverse effects were noted only for four HMs: Herbae pulvis standardisatus, Larrea tridentate, Piper methysticum and Cassia senna. The most severe adverse effects were liver or kidney damage, colon perforation, carcinoma, coma and death.

Seguridad y Precauciones

El uso de Hypericum aegypticum requiere una vigilancia extrema debido a su perfil fitoquímico complejo. En el caso de embarazo y lactancia, no existe evidencia científica suficiente que garantice la seguridad del desarrollo fetal o neonatal; por lo tanto, su uso está estrictamente contraindicado. Los compuestos como los flavonoides y terpenos pueden atravesar la barrera placentaria o excretarse en la leche materna, con riesgos potenciales de toxicidad sistémica en el infante o alteraciones en el desarrollo. Para niños menores de 12 años, la seguridad es desconocida y se desaconseja su administración, ya que los sistemas enzimáticos hepáticos infantiles son inmaduros y pueden reaccar de forma impredecible ante extractos concentrados. En cuanto a las interacciones farmacológicas, el género Hypericum es conocido por su capacidad de inducir enzimas del citocromo P450 (específicamente CYP3A4) y la glicoproteína P. Esto significa que la planta puede acelerar el metabolismo de otros medicamentos, reduciendo su concentración en sangre a niveles sub-terapéuticos.
